This page will contain forms used throughout the year for the band students. Most of these copies are for informational purposes and students should use the originals as distributed by Mr. Schnicke and Mrs. Pendleton as they are sometimes color coded.

ICHAT VOLUNTEER FORM
-If you think that you will be involved in volunteering with the band in any capacity which puts you in direct contact with the students, per school board policy, you must fill out this short form and turn it into the band office so that it can be checked. This goes for ANY and ALL times that you may come into contact with the students. | |
| Band Medicine Form - If you will need any medication (prescription or over the counter) YOUR DOCTOR NEEDS TO SIGN THIS FORM. | |
| Band Health Form - This is a general health form and consent. | |
| Ford Field Waiver - All persons that will step on the field at State Finals, must fill this out. | |
| Band Registration Form - This is the commitment form that is used to register you for a position on the field. | |
